Job DescriptionThe Case Manager II provides case management and consultation services for children and adolescents with mental illness, substance use disorders and/or developmental disabilities and their families.
Also provides supportive counseling, individual and family support, crisis intervention, assessment, Individual Service Plan (ISP) development, psycho-education, and referrals to clients and their families.
Provides assessment services to pregnant women and women with dependent children per agency priority population guidelines protocol.
Develops and maintains working relationships with:
individuals served, families, additional service providers, and other community agencies as needed to ensure quality services.
Works as part of a treatment team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.
Responsibility:
oMaintains a minimum caseload of 24 Medicaid funded mental health case management cases.
oWrites effective, behaviorally specific and measurable Individual Service Plans (ISP) for individuals as required by policy.
Ensures supports and interventions are provided as outlined in the ISP.
At minimum, ISP's are reviewed quarterly and rewritten as needed.
oEnsures that client records meet licensure and reimbursement guidelines.
Accurately completes administrative and other paperwork as required.
Through quality assurance methods, ensures all records will be in compliance with applicable local, state, and federal requirements as they pertain to DBHDS licensure, HIPPA and 42 CFR Part 2 standards and regulations, Medicaid regulations, NRVCS policies and any other applicable regulations and/or guidelines.
oPromotes positive communication and collaboration among clients, families and/or caregivers, team members, and other community resources to provide quality services.
Conducts self in a professional manner at all times.
Participates in team meetings and weekly licensure and administrative supervision, unless absence is excused, completes proper written communication and relays other helpful information as necessary by voice mail and/or e-mail.
Serves on designated teams, committees and work groups.
(Such meetings could include Individualized Education Plan (IEP), school meetings, and/or local Family Assessment Planning Team (FAPT) meetings.
) oProvides direct and related services needed to ensure consumers' needs are addressed.
Performs or assists with bio-psycho-social assessment(s), intake, pre-screening, crisis resolution and/or obtaining additional emergency services as needed.
Provides psycho-education to consumer, families/care providers and other staff as needed.
oProvides for follow through and continuity of care including:
discharge, referral, placement, follow-up appointments and liaison services.
Provides safe transportation in accordance to NRVCS policies and guidelines to individuals served as needed to implement the ISP.
oMeets routinely with supervisor to review status of treatment and address any issues significant to the successful treatment of clients assigned.
Ensures assigned individuals' assessments are completed prior to program enrollment and in accordance with applicable regulations and NRVCS standards.
May be required to perform diagnostic evaluation or testing as needed (if Clinician is trained).
oAssists with crisis resolution and general problem-solving.
Ensures services are provided with respect to the dignity of the individuals served as outlined in the State and NRVCS Human Rights Plans.
Advocates for services on behalf of individuals served and their families/care givers as needed.
Documents and reports suspected violations of Human Rights according to NRVCS policies.
Must be accessible from home, occasionally, to help manage client emergencies and crises as if needed.
oPerforms at the expected levels of individual and team productivity.
Ensures all client records are in compliance with applicable local, state, and federal requirements including guidelines as they pertain to DBHDS Licensure Regulations, Waiver, State Plan and Clinic Options Medicaid.
oEnsures all required certifications and/or licensure are kept up to date.
Attends/provides other training as required to meet personal professional development and/or program needs.
oPerforms other related duties as assigned by Program Supervisor, Program Manager and/or Director.
Qualifications oRequires demonstrated knowledge of case management/social work practices, and resources available to help children and adolescents with serious mental illnesses, substance use disorders and individuals with co-existing disorders.
oWorking knowledge of DSM-V, etiology, symptoms, treatments, prognoses.
Skills in assessment and service plan development is helpful.
oKnowledge and understanding of best practice treatment modalities for serious mental illness, intellectual and developmental, substance use and co-occurring disorders to include behavioral supports and interventions.
oBasic knowledge of human rights, ethical, and legal concerns as they relate to providing case management clinical services.
oWorking knowledge of Medicaid guidelines as they pertain to State Plan, Waiver, and Clinic Option Medicaid.
oMust be able to demonstrate problem-solving, critical-thinking ability.
Should be able to recognize early escalation of symptoms and/or situations and respond appropriately with de-escalation techniques.
oMust be able to exhibit initiative, resourcefulness, and the ability to work independently and as part of a team.
oDemonstrated ability to communicate clearly and concisely (written and oral), working knowledge of computers/word processing.
oMust possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record.
Must maintain acceptable record throughout employment.
oMust have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment; oMust have acceptable drug screen results upon hire.
Education/Experience Required:
oBachelor's Degree in social work, psychology, special education, or human services related field as defined by the VA Board of Counseling is required.
oRegistration as a QMHP or trainee is required.
Must maintain QMHP status and all required trainings/CEUs.
